Text: Thank you very much for completing the questionnaire part of the survey. I would now like to proceed to the part of the survey where we take some measures and collect some samples. Linking data from your questionnaire with health related measurements will help us understand more about your overall health and well-being. If you agree to participate, during this part of the survey, I will be collecting some blood samples, a dried blood spot sample and hair samples. Your blood samples will be analysed for cholesterol (total cholesterol and HDL), triglycerides and HbA1c (glycated haemoglobin) levels which are related to heart disease and diabetes. We will also analyse a number of substances related to nutrition, such as vitamin C, vitamin B1, vitamin B2, vitamin B6, vitamin B12, vitamin A, vitamin E and vitamin D. Your hair sample will be analysed for certain hormones which have been shown to be related to stress including cortisol, cortisone and testosterone. I will be asking you a few questions which are related to each of these measurements. Taking part in this part of the survey is entirely up to you. You can agree to have all or some of the measurements taken. Just let me know as we proceed.
