In order to reduce the difficulty of you entering all of this information manually, I would like to take a picture or screenshot a page from your child's personal child health record (red book or eRedbook) {if GRIDVARIABLES.NCHRESP = 1} children's personal child health records (red books or eRedbook) {if GRIDVARIABLES.NCHRESP > 1}. I would like a picture or screenshot of a review completed by a health worker about your child {if GRIDVARIABLES.NCHRESP = 1} children {if GRIDVARIABLES.NCHRESP > 1}.

If your child is one or older, we would like a picture of their 1 year review page. If they have not had a 1 year review, we would like you to take a picture of their 6-8 week review page. These pictures show the pages we would like pictures of:
