Before I can take any blood, I have to obtain written consent from you. GIVE PARTICIPANT THE CONSENT FORM AND GO THROUGH IT WITH THEM IF NECESSARY. EXPLAIN THE CONSENT PROCEDURE. ASK PARTICIPANT TO SIGN AND DATE CONSENT FORM. DID PARTICIPANT SIGN AND DATE CONSENT FORM?
