Before I can take any blood, I have to obtain written consent from you. COPY NUMBER ONTO CONSENT FORM FOR DRIED BLOOD SPOT SAMPLE ff_pid. GIVE PARTICIPANT THE CONSENT FORM FOR DRIED BLOOD SPOT AND GO THROUGH IT WITH THEM IF NECESSARY. EXPLAIN THE CONSENT PROCEDURE. ASK PARTICIPANT TO SIGN AND DATE DBSCONSCE CONSENT FORM. DID PARTICIPANT SIGN AND DATE CONSENT FORM?
