Before I can take any hair, I have to obtain written consent from you.

and if HAIRWILL = 1 (Willing to provide hair sample)

COPY NUMBER ONTO CONSENT FORM FOR HAIR SAMPLE PID. GIVE PARTICIPANT THE CONSENT FORM FOR HAIR SAMPLE AND GO THROUGH IT WITH THEM IF NECESSARY. EXPLAIN THE CONSENT PROCEDURE. ASK PARTICIPANT TO SIGN AND DATE CONSENT FORM. DID PARTICIPANT SIGN AND DATE CONSENT FORM?
